Абстракт

Background: Accurate assessment of pancreatic beta-cell function in diabetes diagnosis is important for
prognosis and individualized management. C-peptide is a reliable biomarker of endogenous insulin secretion, but data from treatment-naïve South Asian adults are limited. Materials and Methods: Single-center cross-sectional study in Salem, South India, including 100 adults (18-70 years) with newly diagnosed diabetes (< 6 months) and minimal treatment exposure. Fasting and glucagon-stimulated serum C-peptide were measured, with clinical and metabolic profiling and targeted autoantibody testing when indicated. Results: Mean fasting C-peptide was 1.62 ± 0.72 ng/mL and stimulated C-peptide was 2.53 ± 1.01 ng/mL. Low β-cell reserve (< 0.8 ng/mL fasting and/or < 1.8 ng/mL stimulated) was present in 29%. C-peptide showed no clinically meaningful correlation with HbA1c, fasting glucose, BMI, or HOMA indices. Subtype classification suggested 65% T2D-like, 21% possible autoimmune (LADA), and 14% indeterminate. Conclusions: Adults with newly diagnosed diabetes in South India show substantial heterogeneity in β-cell reserve, with nearly one-third having low C-peptide at presentation. Integrating C-peptide (and selective autoantibody testing)
may improve early clinical stratification.
