Майчино-фетални последици на инфекция с вируса на хепатит Е по време на бременност

Инфекцията с вируса на хепатит Е (HEV) по време на бременност значително увеличава риска от неблагоприятни последици за майката и плода. Хепатит Е, особено през втория и третия триместър на бременността, може да доведе до фулминантна чернодробна недостатъчност, следродилен кръвоизлив и майчина смъртност. HEV инфекцията е свързана с по-висока честота на спонтанен аборт, мъртво раждане, преждевременно раждане и вертикално предаване. Уникалността на хепатит Е се състои в трансформацията му от относително самоограничаващ се остър вирусен хепатит в тежко заболяване при бременни жени, главно при инфекция с HEV генотип 1 в ендемични райони. В развиващите се страни както разпространението, така и заболяемостта и смъртността са високи. Механизмите на увреждане на черния дроб по време на бременност все още не са напълно изяснени. Репликация на HEV се наблюдава и в екстрахепатални места, като плацентата.
