Diagnostic and prognostic value of inflammatory and immune-nutritional indices in ovarian cancer

Abstract

Ovarian cancer is among the most lethal gynecologic malignancies, primarily due to late diagnosis. There is growing interest in inflammatory and immune-nutritional indices (NLR, PLR и SII) as accessible and cost-effective (PNI) biomarkers with potential diagnostic and prognostic value. In this study, we found a statistically significant association between all investigated indices and disease stage. With advancing FIGO stage, NLR, PLR, and SII increased, while PNI decreased (p < 0.05). All indices demonstrated good diagnostic performance in distinguishing early from advanced stages, with the highest AUC observed for PNI (AUC = 0.899). The optimal cut-off values determined in our study were: NLR ≥ 3.17; PLR ≥ 214; SII ≥ 1132.9; PNI ≤ 33.9. The indices we evaluated are accessible and informative biomarkers with good diagnostic and prognostic value in ovarian cancer. They can support risk stratification and guide individualized therapeutic strategies. Further multicenter studies are needed to standardize cut-off values and validate their clinical application.
