Периферна нервна блокада като самостоятелен метод на анестезия при оперативни интервенции на долен крайник

Отразявайки нарастващото внимание към болката, Американското дружество по болка я определя като 5-и жизненоважен показател и декларира, че успехът или неуспехът на операцията се определя от управлението на следоперативната болка. Периферните нервни блокади с локални анестетици са част от периоперативния режим на аналгезия в продължение на много години. Много проучвания са доказали тяхната ефективност. Тези проучвания показват, че пациентите, които получават периферни нервни блокади, изпитват намалена следоперативна болка, имат по-малка нужда от аналгезия и съобщават за по-голямо удовлетворение от управлението на болката. Могат ли обаче тези блокади да се използват и като самостоятелен метод за анестезия също толкова ефективно? Цел и методи: Целта на нашето проучване е да се провери дали периферните нервни блокади са надежден и успешен метод за анестезия, използван самостоятелно. Проведено е проспективно рандомизирано проучване, включващо 60 пациенти, при които е осъществена периферна нервна блокада под ултразвуков контрол като самостоятелен метод на анестезия за извършване на хирургична интервенция на долния крайник в операционните зали на Катедра „Ортопедия, травматология и реконструктивна хирургия“ във Военномедицинска академия, МБАЛ – София. Направихме статистически анализ на данните относно процента на успех на периферната нервна блокада, необходимостта от допълнителни медикаменти или седация, необходимостта от преминаване към друг вид анестезия, продължителността на хирургичната интервенция и удовлетвореността на пациентите. Заключение: Периферната нервна блокада се оказва надежден и успешен метод за анестезия, използван в комбинация с лека седация на пациентите. Като самостоятелна анестетична техника, дори при използването на ехограф, степента на успеваемост значително намалява в сравнение с използването му в комбинация с допълнителна медикация
