Абстракт

Инкретиновата ос играе централна роля в регулацията на глюкозния метаболизъм и енергийния баланс, като нейната дисрегулация е ключов патофизиологичен механизъм при захарен диабет тип 2, затлъстяване и свързаните с тях кардиометаболитни заболявания. Глюкагон-подобният пептид-1 (GLP-1) и глюкозозависимият инсулинотропен полипептид (GIP) действат чрез рецепторно медиирани механизми, които засягат не само панкреасната функция, но и сърдечносъдовата система, мастната тъкан, черния дроб и централната нервна система. Съвременните терапевтични подходи, включващи GLP-1 рецепторни агонисти и dual incretin терапии, демонстрират широк спектър от ефекти, надхвърлящи гликемичния контрол. Настоящата обзорна статия има за цел да обобщи молекулярните и системните механизми на действие на тези терапии и тяхната роля в кардиометаболитните заболявания.
